From what I remember, at our churches it seemed that most of these were unwritten expectations and varied some.  Mostly I remember no long hair for men, no piercings (it was only ears at that time)  Tattoos were acceptable if they were obtained in a life of sin but never any new ones, the rest seemed to be ok unless it was sunday morning (The northwest was too cold to wear shorts or sandals to church anyway.)  Our pastor at the last church I went to used to get his hair permed.
