Clean shaven- that means not even a 5:00 shadow

No dying the hair

no perms

No shaving the head

No earrings, tongue or nose rings

No tats

No tanktops or sleaveless shirts

No shorts

No sandals

Shoes and socks must be worn

Shirts should be buttoned up to the next to top button if not wearing a tie
I've also heard:
No pleats in the front of pants (or that they had to wear pleats, depends on the church)
No bright colors at church
No hankies in suit pockets
No tie clips (but oh, you have to wear a tie)
No more than 3 buttons on a suit coat (can't go a-lustin' after them buttons!)
Dress shoes can't be patton leather or shiny or have a fringe or tassle (depending on what's stylish, this can be hard)
Only long sleeves
only white button down the front dress shirts at church
no cuff links
only long pants at all times-no shorts, no pants that come below the knee but not to the ankle
Very restrictive hair style limitations
No hair spray or styling gel or other hair products to keep hair in place--it shouldn't be that long!
No shaving the head, just the face
Sideburns should come to that little pointy place mid front of your ear, no lower.

And more. Men didn't look as abnormal for what they wore, weren't as restricted in where they could work, and could be warm in winter, but there were quite a few weird rules for them. And many times they didn't know the rules til they broke one and the pastor publicly got on their case. Generally he made the rules up as he went.
